Parent's Night Out

Liability & Permission

I, the undersigned, give permission for my child to participate in Parent Night Out at Orange Daisy Bilingual School.

I understand and agree to:

1. Children will be supervised by staff (ratio approx. 1:6).

2. Activities may include crafts, games, and snacks. Participation involves normal childcare activity risks.

3. My child is healthy and free of contagious illness.

4. Staff may seek medical treatment in case of emergencies. Parent/Guardian is responsible for all medical costs.

5. Liability release: I release Orange Daisy Bilingual School and staff from liability for accidents or injuries, except in gross negligence.

6. Behavior policy: Staff may contact me to pick up my child if safety rules are violated.

7. Pick-up: Children must be picked up promptly at 9:00 PM. Late pick-up fee applies. ($2 per minute)

Payment & Registration

• Full payment of $60 is required to secure the spot.

• $50 per additional child.

• Spots are limited to 18 children.

• Payment confirms enrollment.
